    Submitting a Regularly Scheduled Series Application


    Minimal Requirements
    • Letter of Request on letterhead
    • PLEASE NOTE: If it is a collaborative program, any partner or group that is listed on application, must send a letter stating their participation and acknowledging their role with the grant.
    • Budget Schedule (xls)
      • Please note that we will also accept the standardized budget template found on the Alliance for CME website
    • Proposal (doc)
    • Agenda and/or List of Topics (drafts accepted)
    • Program Brochure (drafts accepted)
